| Abstrak (Bhs. Indonesia) | Metotreksat (MTX) adalah salah satu disease-modifying antirheumatic drugs (DMARDs), suatu agen antiinflamasi dan imunosupresan yang menjadi first line terapi rheumatoid arthritis (RA). Keberhasilan terapi MTX untuk RA ditentukan oleh ketepatan dosis dan monitoring. Tujuan penelitian ini adalah untuk mengevaluasi penggunaan MTX pada pasien RA di Rumah Sakit Emanuel Klampok berdasarkan kriteria yang meliputi indikasi, indikator proses, komplikasi, dan indikator hasil. Penggunaan MTX dievaluasi berdasarkan kriteria yang dibuat melalui studi pustaka dari jurnal-jurnal ilmiah dan text book. Analisis data dilakukan secara deskriptif evaluatif menggunakan data rekam medik pasien RA yang menggunakan MTX yaitu sejumlah 13 pasien rawat inap dan 27 pasien rawat jalan yang kemudian dibandingkan dengan kriteria yang telah dibuat. Hasil penelitian menunjukkan ketepatan indikasi 100%. Pasien yang memiliki faktor risiko gangguan GI, hepatotoksik, dan toksisitas bone marrow berturut-turut sejumlah 35 pasien (87,5%), 19 pasien (47,5%), dan 15 pasien (37,5%). Monitoring hematologi, SGPT, dan SCr berturut-turut pada 3 pasien (7,5%), 1 pasien (2,5%), dan 1 pasien (2,5%). Asam folat hanya diberikan pada 1 pasien (2,5%). Pemberian MTX dengan dosis yang tepat sejumlah 32 pasien (80%), dosis tidak tepat dengan ClCr 61-80 mL/menit sejumlah 3 pasien (7,5%), ClCr 51-60 mL/menit sejumlah 2 pasien (5%), ClCr 10-50 mL/menit sejumlah 1 pasien (2,5%), dan SGPT > 3 nilai normal sejumlah 2 pasien (5%). Interaksi MTX dengan NSAID sejumlah 35 pasien (87,5%) dan dengan agen hepatotoksik sejumlah 19 pasien (47,5%). Komplikasi terjadi pada 7 pasien (17,5%) berupa gangguan gastrointestinal dan 1 pasien (2,5%) berupa sirosis. Indikator hasil berupa berkurangnya keluhan klinis seperti nyeri dan kaku terjadi pada 10 pasien (25%) dan pasien yang membaik sejumlah 2 pasien (5%). |

Inggris) | Methotrexate (MTX), one of a disease-modifying antirheumatic drugs (DMARDs), is the first line therapy for rheumatoid arthritis (RA) as an antiinflammatory and immunosuppressant agent. Therapeutic efficacy of MTX is 2 determined by accuracy of dosing and monitoring. The purpose of this study was to evaluate the use of MTX in patients with rheumatoid arthritis at Rumah Sakit Emanuel Klampok based on criteria, including the indication, process indicators, complication, and outcome indicators. The medical record from 13 inpatients and 27 outpatients who used MTX were compared with the criteria. The results of this study demonstrated that all of the patients had appropriately indications to use MTX. Patients with risk factors that lead to GI disorders, hepatotoxicity, and bone marrow toxicity were 35 patients (87,5%), 19 patients (47,5%), and 15 patients (37,5%) respectively. Monitoring of haematology, SGPT, and SCr were in 3 patients (7,5%), 1 patient (2,5%), and 1 patient (2,5%) respectively. Only 1 patient (2,5%) received folic acid. There were 32 patients (80%) used MTX with the correct dosage, meanwhile incorrect dosage was showed in 3 patients (7,5%) with ClCr 61-80 mL/minute, 2 patients (5%) with ClCr 51-60 mL/minute, 1 patient (2,5%) with ClCr 10-50 mL/minute, and 2 patients (5%) with SGPT > 3 normal value. The interaction with NSAID was happened in 35 patients (87,5%) and the interaction with hepatotoxicity agents in 19 patients (47,5%). Complication occurred in 7 patients (17,5%) with effects that occur were GI disorders and 1 patient with chirrosis. There were 10 patients (25%) with clinical complaints reduced and 2 patients (5%) with the better condition. MTX use criteria in RA patients was developed and evaluation was conducted by comparing the data and criteria. |
