| Judul Artikel | Pengaruh Berbagai Macam Sleep Deprivation terhadap Leptin, Ghrelin, dan Histopatologi Lambung Tikus Wistar Jantan |
| Abstrak (Bhs. Indonesia) | Latar Belakang: Sleep deprivation adalah kurangnya durasi tidur yang dapat berupa total sleep deprivation (TSD) dan paradoxical sleep deprivation (PSD). Kondisi ini mempengaruhi regulasi hormon leptin dan ghrelin melalui aktivasi sumbu hypothalamic-pituitary-adrenal (HPA axis), stres oksidatif, dan gangguan regulasi melatonin, serta dapat merusak mukosa lambung akibat pelepasan sitokin proinflamasi (IL-8, IL-6). Sleep recovery menurunkan aktivitas HPA axis dan stres oksidatif, meningkatkan melatonin, serta menormalkan irama sirkadian. Tujuan: Mengetahui pengaruh berbagai jenis sleep deprivation terhadap kadar leptin, ghrelin, dan gambaran histopatologi lambung tikus putih jantan galur Wistar, serta mengevaluasi efek sleep recovery. Metode: Penelitian true experimental dengan post-test only with control group design pada 25 tikus Wistar jantan yang dibagi menjadi 5 kelompok: kontrol, TSD, TSD+SR, PSD, dan PSD+SR. Kadar leptin dan ghrelin diukur dengan ELISA, sedangkan histopatologi lambung dinilai menggunakan skor klasifikasi Sydney yang dimodifikasi. Analisis menggunakan One-way ANOVA untuk leptin, Welch ANOVA untuk ghrelin, dan Kruskal Wallis untuk skor histopatologi lambung. Hasil: Kadar leptin tertinggi pada kontrol (4,20 ± 0,56 ng/mL) dan terendah pada TSD (1,23 ± 0,36 ng/mL); kadar ghrelin tertinggi pada TSD (7462,6 ± 1230,3 ng/L) dan terendah pada kontrol (3946,2 ± 261,8 ng/L). Sleep deprivation menurunkan leptin dan meningkatkan ghrelin secara signifikan. Gambaran histopatologi lambung menunjukkan gambaran normal pada kelompok kontrol (skor 3) dan kerusakan paling berat pada PSD (skor 9) dan PSD+SR (skor 9). Sleep recovery meningkatkan leptin, menurunkan ghrelin, serta memperbaiki sebagian gambaran histopatologi lambung, namun belum memulihkan integritas mukosa sepenuhnya. Kesimpulan: Sleep deprivation menurunkan leptin, meningkatkan ghrelin, dan merusak mukosa lambung, dengan kerusakan terberat pada PSD dan PSD+SR. Sleep recovery memperbaiki kadar hormon secara signifikan pada leptin, namun efek pada pemulihan kadar ghrelin dan perbaikan mukosa lambung tidak signifikan. | Abtrak (Bhs. Inggris) | Background: Sleep deprivation refers to insufficient sleep duration, which can be categorized into total sleep deprivation (TSD) and paradoxical sleep deprivation (PSD). This condition affects the regulation of leptin and ghrelin through activation of the hypothalamic–pituitary–adrenal (HPA) axis, oxidative stress, and melatonin dysregulation, and can damage the gastric mucosa via the release of proinflammatory cytokines (IL-8, IL-6). Sleep recovery reduces HPA axis activity and oxidative stress, increases melatonin production, and restores circadian rhythm. Objective: To determine the effects of various types of sleep deprivation on leptin, ghrelin, and gastric histopathology in male Wistar rats, and to evaluate the impact of sleep recovery. Methods: This true experimental study used a post-test only control group design on 25 male Wistar rats divided into five groups: control, TSD, TSD+SR, PSD, and PSD+SR. Leptin and ghrelin levels were measured using ELISA, while gastric histopathology was assessed using a modified Sydney classification scoring system. One-way ANOVA was used for leptin, Welch ANOVA for ghrelin, and Kruskal–Wallis for gastric histopathology scores. Results: The highest leptin level was found in the control group (4.20 ± 0.56 ng/mL) and the lowest in the TSD group (1.23 ± 0.36 ng/mL); the highest ghrelin level was in the TSD group (7462.6 ± 1230.3 ng/L) and the lowest in the control group (3946.2 ± 261.8 ng/L). Sleep deprivation significantly decreased leptin and increased ghrelin. Gastric histopathology showed more severe damage in PSD and PSD+SR groups. Sleep recovery increased leptin, decreased ghrelin, and partially improved gastric histopathology but did not fully restore mucosal integrity. Conclusion: TSD and PSD lead to decreased leptin, increased ghrelin, and gastric mucosal damage, with PSD causing more severe effects. Sleep recovery improves hormonal and histological parameters, but mucosal recovery requires more time than hormonal restoration.
