| Abstrak (Bhs. Indonesia) | Latar Belakang: Antihipertensi seringkali digunakan secara kombinasi. Hal ini dapat meningkatkan potensi terjadinya interaksi obat yang dapat memberikan reaksi terhadap pengobatan, serta dapat mempengaruhi efek dan keamanan tiap obat. Pasien usia lanjut lebih berisiko terjadinya masalah terkait penggunaan obat karena menurunnya fungsi organ, sehingga meningkatkan risiko komplikasi penyakit lain. Penelitian ini bertujuan untuk mengidentifikasi potensi interaksi antihipertensi dengan obat lain pada resep pasien lansia di Apotek Karya Sehat. Metodologi: Penelitian ini menggunakan pendekatan cross sectional dengan desain penelitian observasional. Data yang digunakan adalah resep di Apotek Karya Sehat periode Januari-Maret 2024. Potensi interaksi akan diidentifikasi menggunakan database dari Lexidrug by UpToDate. Hasil Penelitian: Dari 413 sampel yang didapatkan, terdapat 122 resep (29,53%) memiliki potensi interaksi dengan total potensi interaksi sebanyak 184 potensi yang terdiri dari 7 potensi (3,80%) interaksi minor, 172 potensi (93,48%) interaksi moderat, dan 5 potensi (2,72%) interaksi mayor. Berdasarkan mekanisme didapatkan hasil 85 potensi (46,20%) secara farmakokinetik, 90 potensi (48,91%) secara farmakodinamik, dan 9 potensi (4,89%) yang tidak diketahui. Potensi interaksi terbanyak merupakan potensi interaksi antara amlodipin dengan simvastatin yang berinteraksi secara farmakokinetik pada tingkat moderat sebanyak 76 potensi interaksi. Kesimpulan: Terdapat sejumlah 29,53% potensi interaksi obat pada resep antihipertensi pasien lansia yang dapat mempengaruhi efektivitas dan keamanan obat. Interaksi obat terbanyak terjadi pada fase farmakodinamik dan pada tingkat keparahan moderat. Penanganan interaksi dimulai dari edukasi terhadap pasien, pemantauan kondisi klinis pasien, modifikasi terapi, maupun rekomendasi penggantian terapi. |

This can increase the potential for drug interactions that can cause reactions to treatment and can affect the effects and safety of each drug. Elderly patients are more at risk of problems related to drug use due to decreased organ function, thereby increasing the risk of complications from other diseases. This study aims to identify potential interactions between antihypertensives and other drugs prescribed at the Karya Sehat Pharmacy. Methodology: This research uses a cross sectional approach with an observational research design. The data used are prescriptions at the Karya Sehat Pharmacy for the period January-March 2024. Potential interactions will be identified using the database from Lexidrug by UpToDate. Research Results: There are 122 recipes (29.53%) that have interaction potential with a total of 184 potential interactions consisting of 7 potential (3.80%) minor interactions, 172 potential (93.48%) moderate interactions, and 5 potential (2.72%) major interactions. Based on the mechanism, the results obtained were 85 potencies (46.20%) pharmacokinetically, 90 potencies (48.91%) pharmacodynamically, and 9 potencies (4.89%) were unknown. The most potential interactions are the potential interactions between amlodipine and simvastatin which interact pharmacokinetically at a moderate level with 76 potential interactions. Conclusion: There are 29.53% potential drug interactions in antihypertensive prescriptions for elderly patients which can affect the effectiveness and safety of the drug. Most drug interactions occur in the pharmacodynamic phase and at moderate severity levels. Handling interactions starts with educating the patient, monitoring the patient's clinical condition, modifying therapy, and recommending changing therapy. |
