| Abstrak (Bhs. Indonesia) | Penggunaan obat-obat yang bertarget pada reseptor histamin ini banyak digunkan sebagai obat di luar indikasi. Presentase peresepan off-label pada obat antihistamin mencapai 32.3%. Penelitian ini bertujuan untuk mengetahui apa saja obat yang bekerja pada reseptor histamin yang memiliki indikasi offlabel, mekanisme obat pada indikasi offlabel, dan peringatan serta efek samping yang mungkin ditimbulkan pada penggunaan obat. Penelitian ini menggunakan metode narrative review dengan telaah literatur yang diambil dari PubMed. Sebanyak 50 artikel yang digunakan merupakan artikel berbahasa Inggris, tanpa batasan tahun. Hasil telaah literatur diperoleh bahwa terdapat 7 obat yang bekerja pada reseptor histamin H1 (CTM, difenhidramin, siproheptadin, prometazin, setirizin, fexofenadin, dan desloratadin) dan 3 obat (famotidin, simetidin, dan nizatidin) yang bekerja pada reseptor H2 memiliiki indikasi offlabel. Mekanisme aksi obat bervariasi yaitu dengan penghambatan reseptor H1 & H2, dopamin, muskarinik, dan serotonin serta mekanisme lain terkait struktur obat. Terdapat studi efikasi obat pada indikasi offlabel. CTM sebagai terapi mabuk perjalanan ; difenhidramin terapi anestesi lokal, mual muntah saat kehamilan, keracunan scombroid dan vertigo ; siproheptadin sebagai peningkat nafsu makan, terapi sindrom serotonin dan spatisitas; prometazin sebagai terapi mual muntah saat kehamilan; desloratadin dan fexofenadin pada terapi urtikaria; famotidin pada terapi pencegahan aspirasi setelah anestesi, premedikasi infus kemoterapi, mastositosis, dan profilaksis stress ulcer; simetidin pada terapi sistitis interstisal; dan nizatidin pada eradikasi h.pylori. Terdapat hal-hal yang harus diperhatikan pada penggunaan obat -obat tersebut, seperti efek samping seperti efek sedasi, defisiensi vit B12, perpanjangan interval QTc, peringatan pada pasien khusus (pediatri atau geriatri) dan juga penyakit penyerta. |

| Abtrak (Bhs. Inggris) | The use of drugs that are targeted at the histamine receptor is widely used as a drug outside of indications. The percentage of off-label prescriptions for antihistamines reached 32.3%. This study aims to find out what drugs act on histamine receptors that have offlabel indications, the mechanism of drugs on offlabel indications, and warnings and side effects that may be caused by drug use. This study uses a narrative review method with a literature review taken from PubMed. A total of 50 articles used are articles in English, with no year limitation. The results of the literature review showed that there are 7 drugs that act on histamine H1 receptors (CTM, diphenhydramine, cyproheptadine, promethazine, cetirizine, fexofenadine, and desloratadine) and 3 drugs (famotidine, cimetidine, and nizatidine) that act on H2 receptors have offlabel indications. Mechanisms of action of drugs vary, namely by inhibition of H1 & H2, dopamine, muscarinic, and serotonin receptors and other mechanisms related to drug structure. There are drug efficacy studies on offlabel indications. CTM as a motion sickness therapy; diphenhydramine for local anesthetic therapy, nausea and vomiting in pregnancy, scombroid poisoning and vertigo; cyproheptadine as appetite enhancer, treatment of serotonin syndrome and spasticity; promethazine as a treatment for nausea and vomiting in pregnancy; desloratadine and fexofenadine in the treatment of urticaria; famotidine in the prevention of aspiration after anesthesia, chemotherapy infusion premedication, mastocytosis, and stress ulcer prophylaxis; cimetidine in the treatment of interstitial cystitis; and nizatidine on H.pylori eradication. There are things that must be considered when using these drugs, such as side effects such as sedation, vitamin B12 deficiency, QTc prolongation, warnings for special patients (pediatric or geriatric) and comorbidities. |
