| Abstrak (Bhs. Indonesia) | Penatalaksanaan terapi farmakologi DM tipe 2 sering berpotensi mengalami drug related problems (DRPs). Checklist adalah acara yang paling dan efisian dalam mengidentifikasi DRPs, namun belum pernah ada pengembangannya di Indonesia. Penelitian ini bertujuan untuk mengembangkan checklist yang dapat mempermudah apoteker dalam mendeteksi DRPs pada pasien DM tipe 2. Penelitian non eksperimental deksriptif ini dilakukan dengan metode studi literatur terapi DM tipe 2 yang disesuaikan dengan pengobatan di Indonesia. Literatur utama yang digunakan berupa guideline terapi DM tipe 2 (PERKENI, ADA, AACE), buku Drug Information handbook (2015) dan Drug Interaction Facts (2012), Formularium Nasional, monografi Medscape, Up To Date, dan Drug Interaction Checker dari Medscape. Checklist kemudian diuji keterbacaannya dengan uji readibilitas oleh 2 apoteker klinis di rumah sakit dan 3 apoteker komunitas di apotek. Penelitian ini mampu menghasilkan checklist yang berupa daftar pertanyaan disertai pilihan jawaban dan interpretasi DRPs yang dilengkapi dengan panduan. Checklist yang dihasilkan mampu mengidentifikasi DRPs pemilihan obat (duplikasi dan kontraindikasi), permasalahan dosis obat, ESO, interaksi obat, dan permasalahan penggunaan obat (waktu minum obat). Hasil uji readibilitas menunjukkan bahwa checklist ini dapat terbaca. |

| Abtrak (Bhs. Inggris) | Management therapy for type 2 diabetes mellitus often potentially cause drug related problems (DRPs). The checklist is the most efficient and efficient program in identifying DRPs, but there has never been any development in Indonesia. This research aims to develop a checklist that facilitates pharmacists to detect DRPs in type 2 diabetes mellitus patients. An experimental descriptive research was done with study of therapy for type 2 diabetes mellitus literature that adjusted to/compatible with therapy in Indonesia. The prior literatures were guideline therapy for type 2 diabetes mellitus (PERKENI, ADA, AACE), books such as Drug Information Handbook (2015) and Drug Interaction Facts (2012), Formularium Nasional, drug monography from Medscape, Up To Date, and drug interaction checker from Medscape. Checklist was tested its readability by 3 clinical pharmacists in hospital and 3 community pharmacists in pharmacy. This research delivered checklist which was contained a list of questions with choice of answer and DRPs interpretation that was equipped with guidance. The checklist was able to identify drug selection DRPs (duplication and contraindication), problems of drug dosage, ADR, and drug interaction, problems of drug use (time to take medicine). The result of readability was shown that the checklist was readable. |
