ABSTRAK PENGARUH AUTOMATIC BICYCLE FOR EXTREMITY MUSCLE TRAINING (ATOMIK) TERHADAP KEKUATAN OTOT PASIEN STROKE ISKEMIK DI RSUD AJIBARANG Soni Handoyo¹, Sidik Awaludin², Arif Setyo Upoyo³ Latar Belakang: Stroke iskemik dapat menyebabkan kelemahan otot ekstremitas atas dan bawah. Automatic Bicycle for Extremity Muscle Training (Atomik) dikembangkan sebagai alat bantu latihan otot ekstremitas atas dan bawah pada pasien stroke iskemik. Tujuan Penelitian: Menganalisis pengaruh Automatic Bicycle for Extremity Muscle Training (Atomik) terhadap perubahan kekuatan otot ekstremitas atas dan bawah pasien stroke iskemik. Metode Penelitian: Tahap pertama menggunakan metode Research and Development (R&D) dengan prototype Automatic Bicycle for Extremity Muscle Training (Atomik). Tahap kedua menggunakan desain true experimental randomized kontrol group pretest-posttest design. Responden berjumlah 66 pasien stroke iskemik, terdiri atas 33 kelompok intervensi dan 33 kelompok kontrol. Kekuatan otot diukur pada interval waktu (pretest, minggu ke-1, ke-2, ke-3, dan ke-4). Analisis data menggunakan uji Friedman, pairwise comparisons dengan koreksi Bonferroni dan Mann-Whitney. Hasil: Content Validity Index (CVI) sebesar 1,00 menunjukkan validitas isi yang baik, sedangkan Intraclass Correlation Coefficient (ICC) sebesar 0,962 menunjukkan reliabilitas yang tinggi. Uji Friedman menunjukkan adanya perbedaan kekuatan otot antarwaktu pada ekstremitas atas dan bawah dengan p < 0,001. Uji Mann-Whitney menunjukkan terdapat perbedaan perubahan kekuatan otot yang signifikan antara kelompok intervensi dan kontrol pada ekstremitas bawah (p = 0,019), sedangkan pada ekstremitas atas tidak terdapat perbedaan yang signifikan (p = 0,053). Kesimpulan: Automatic Bicycle for Extremity Muscle Training (Atomik) menunjukkan adanya perbedaan perubahan kekuatan otot ekstremitas bawah antara kelompok intervensi dan kontrol. Namun, tidak terdapat perbedaan perubahan kekuatan otot ekstremitas atas yang signifikan. ABSTRACT THE EFFECT OF AUTOMATIC BICYCLE FOR EXTREMITY MUSCLE TRAINING (ATOMIK) ON MUSCLE STRENGTH IN PATIENTS WITH ISCHEMIC STROKE AT AJIBARANG REGIONAL GENERAL HOSPITAL Soni Handoyo¹, Sidik Awaludin², Arif Setyo Upoyo³ Background: Ischemic stroke can cause muscle weakness in the upper and lower extremities. The Automatic Bicycle for Extremity Muscle Training (Atomik) was developed as an assistive device for upper- and lower-extremity muscle training in patients with ischemic stroke. Objective: To analyze the effect of the Automatic Bicycle for Extremity Muscle Training (Atomik) on changes in upper- and lower-extremity muscle strength in patients with ischemic stroke. Methods: The first stage used a Research and Development (R&D) method to develop a prototype of the Automatic Bicycle for Extremity Muscle Training (Atomik). The second stage employed a true experimental randomized controlled group pretest-posttest design. A total of 66 patients with ischemic stroke participated in the study, comprising 33 patients in the intervention group and 33 patients in the control group. Muscle strength was measured at multiple time points (pretest, week 1, week 2, week 3, and week 4). Data were analyzed using the Friedman test, pairwise comparisons with Bonferroni correction, and the Mann-Whitney U test. Results: The Content Validity Index (CVI) of 1.00 indicated good content validity, while an Intraclass Correlation Coefficient (ICC) of 0.962 indicated high reliability. The Friedman test demonstrated significant differences in muscle strength across time points in both the upper and lower extremities (p < 0.001). The Mann-Whitney U test showed a significant difference in changes in lower-extremity muscle strength between the intervention and control groups (p = 0.019), whereas no significant difference was found in upper-extremity muscle strength (p = 0.053). Conclusion: The Automatic Bicycle for Extremity Muscle Training (Atomik) demonstrated a significant difference in changes in lower-extremity muscle strength between the intervention and control groups. However, no significant difference was observed in changes in upper-extremity muscle strength.
