Studi Kasus Pengaruh Foot Massage terhadap Skala Nyeri dan Status Hemodinamik Pada Pasien Terpasang Ventilator Mekanik di Ruang Intensive Care Unit (ICU)

Latar Belakang: Pasien yang dirawat di ruang Intensive Care Unit, khususnya yang terpasang ventilator mekanik, sering mengalami nyeri dan ketidakstabilan status hemodinamik. Nyeri yang tidak terkelola dengan baik dapat memicu aktivasi sistem saraf simpatis sehingga meningkatkan tekanan darah, denyut jantung, dan frekuensi napas. Oleh karena itu, diperlukan intervensi nonfarmakologis yang aman dan efektif, yaitu foot massage, yang berpotensi memberikan efek relaksasi dan menstabilkan kondisi fisiologis pasien. Metodologi: Penelitian ini menggunakan desain studi kasus dengan penerapan evidence based practice pada dua pasien di ruang Intensive Care Unit yang terpasang ventilator mekanik. Intervensi berupa foot massage dilakukan selama 20 menit dalam sehari. Pengukuran skala nyeri menggunakan Critical-Care Pain Observation Tool, sedangkan status hemodinamik meliputi tekanan darah, mean arterial pressure, frekuensi nadi, frekuensi napas, dan SpO₂. Observasi dilakukan sebelum intervensi dan selama 2 jam setelah intervensi setiap 15 menit. Pemberian intervensi dilakukan pada tanggal 11 dan 12 Desember 2025. Hasil: Hasil penelitian menunjukkan kecenderungan penurunan nyeri pada kedua pasien setelah foot massage, terutama pada menit ke-30–60, disertai perbaikan hemodinamik (tekanan darah, mean arterial pressure, nadi, dan napas) yang cenderung menurun lalu stabil. Sementara itu, nilai SpO₂ tidak menunjukkan perubahan signifikan dan tetap dalam batas normal. Kesimpulan: Pemberian foot massage berpotensi menurunkan skala nyeri dan memperbaiki status hemodinamik pada pasien dengan ventilator mekanik di ruang Intensive Care Unit RSUD Prof. Dr. Margono Soekarjo.

Background: Patients admitted to the Intensive Care Unit, particularly those on mechanical ventilation, often experience pain and hemodynamic instability. Poorly managed pain can trigger activation of the sympathetic nervous system, thereby increasing blood pressure, heart rate, and respiratory rate. Therefore, a safe and effective nonpharmacological intervention specifically foot massage is needed, as it has the potential to provide a relaxing effect and stabilize the patient’s physiological condition. Methodology: This study employed a case study design with the application of evidence-based practices on two patients in the Intensive Care Unit who were on mechanical ventilation. The intervention, which consisted of foot massage, was performed for 20 minutes daily. Pain was measured using the Critical-Care Pain Observation Tool, while hemodynamic status included blood pressure, mean arterial pressure, pulse rate, respiratory rate, and SpO₂. Observations were made before the intervention and every 15 minutes for 2 hours after the intervention. The intervention was conducted on December 11 and 12, 2025. Results: The study results showed a trend toward decreased pain in both patients following foot massage, particularly between 30 and 60 minutes, accompanied by hemodynamic improvements (blood pressure, mean arterial pressure, pulse, and respiratory rate) that initially decreased and then stabilized. Meanwhile, SpO₂ levels did not show significant changes and remained within normal limits. Conclusion: Foot massage has the potential to reduce pain levels and improve hemodynamic status in patients on mechanical ventilation in the Intensive Care Unit at Prof. Dr. Margono Soekarjo Regional General Hospital.
