Analisis Drug-Related Problems (DRPs) Penggunaan Antipsikotik pada Pasien Skizofrenia di Rumah Sakit Umum Daerah Prof. Dr. Margono Soekarjo

Latar Belakang: Skizofrenia merupakan gangguan kejiwaan heterogen dengan gejala psikotik, gangguan berpikir, dan fungsi psikososial yang umumnya ditangani dengan antipsikotik, sering dalam bentuk kombinasi sehingga berisiko menimbulkan polifarmasi dan drug-related problems (DRPs). Tingginya DRPs dapat menurunkan efektivitas hingga menyebabkan kegagalan terapi. Penelitian ini bertujuan mengidentifikasi pola penggunaan antipsikotik serta DRPs aktual maupun potensial pada pasien skizofrenia di RSUD Prof. Dr. Margono Soekarjo. Metodologi: Penelitian retrospektif dilakukan pada pasien skizofrenia rawat jalan yang menerima antipsikotik minimal 3 bulan di RSUD Prof. Dr. Margono Soekarjo, menggunakan teknik total sampling. DRPs kategori masalah dan penyebab dianalisis secara deskriptif berdasarkan Pharmaceutical Care Network Europe V.9.1. Hasil Penelitian: Dari 85 pasien skizofrenia yang mendapatkan antipsikotik, 64 pasien (75,29%) mendapatkan kombinasi ≥2 antipsikotik, dengan kombinasi terbanyak yaitu risperidon dan klozapin pada 15 pasien (17,65%). Pada kategori masalah, ditemukan 78 kejadian DRPs, dengan prevalensi tertinggi berupa efek terapi tidak optimal yang terjadi pada 40 pasien (51,28%). Pada kategori penyebab, ditemukan 202 kejadian DRPs, dengan prevalensi tertinggi berupa kombinasi tidak tepat antara obat dengan obat yang terjadi pada 85 pasien yang diteliti (42,08%). Kesimpulan: DRPs pada pasien skizofrenia ditemukan pada kategori masalah dan penyebab terutama pada kategori efek obat tidak optimal dan kombinasi obat yang tidak tepat sehingga asuhan kefarmasian diperlukan untuk melakukan monitoring terhadap perkembangan penyakit pasien dan mengoptimalkan pengobatan yang rasional.

Background: Schizophrenia is a heterogeneous mental disorder characterized by psychotic symptoms, thought disorders, and impaired psychosocial functioning, which is generally treated with antipsychotics often in combination thereby posing a risk of polypharmacy and drug-related problems (DRPs). A high incidence of DRPs can reduce treatment effectiveness and even lead to treatment failure. This study aims to identify patterns of antipsychotic use as well as actual and potential DRPs among patients with schizophrenia at Prof. Dr. Margono Soekarjo Hospital. Methods: A retrospective study was conducted on outpatients with schizophrenia who had been receiving antipsychotics for at least 3 months at Prof. Dr. Margono Soekarjo Hospital, using total sampling. DRPs categorized by problem and cause were analyzed descriptively based on Pharmaceutical Care Network Europe V.9.1. Results: Of the 85 patients with schizophrenia receiving antipsychotics, 64 patients (75.29%) received a combination of ≥2 antipsychotics, with the most common combination being risperidone and clozapine (17.65%). In the "problem" category, 78 DRPs events were identified, with the highest prevalence being suboptimal therapeutic effects (51.28%). In the "cause" category, 202 DRPs events were identified, with the highest prevalence being inappropriate drug-drug combinations (42.08%). Conclusion: DRPs in patients with schizophrenia were identified in the categories of problems and causes, particularly in the categories of suboptimal drug effects and inappropriate drug combinations; therefore, pharmaceutical care is necessary to monitor the progression of the patients’ disease and optimize rational treatment.
