KAJIAN HUBUNGAN KESESUAIAN PENGGUNAAN ANTIBIOTIK DEFINITIF TERHADAP LUARAN KLINIS PADA PASIEN SEPSIS DEWASA DI RSUD PROF. DR. MARGONO SOEKARJO PURWOKERTO

Latar Belakang: Sepsis adalah respons inflamasi sistemik akibat infeksi yang dapat menyebabkan kegagalan organ dan kematian. Terapi antibiotik definitif penting dalam penanganan sepsis, namun tingkat kesesuaiannya masih rendah. Metode: Penelitian ini bersifat observasional retrospektif dengan total sampling terhadap pasien sepsis dewasa di RSUD Prof. Dr. Margono Soekarjo periode Januari–Desember 2024. Evaluasi kesesuaian antibiotik dilakukan berdasarkan hasil kultur, sensitivitas, dan guideline PNPK 2017, IDSA 2020, serta DIH 2021. Analisis menggunakan uji Chi-Square (p=0,05). Hasil: Dari 77 regimen dengan 133 antibiotik, 64,95% berupa terapi kombinasi. Kesesuaian terhadap hasil kultur 41,66%, guideline 37,66%, dan dosis 75,94%. Perbaikan luaran klinis lebih tinggi pada terapi yang sesuai, namun tidak signifikan secara statistik (p>0,05). Kesimpulan: Kesesuaian antibiotik definitif terhadap kultur dan guideline masih rendah. Evaluasi rutin dibutuhkan untuk meningkatkan luaran klinis dan mencegah resistensi antibiotik.

Background: Sepsis is a systemic inflammatory response to infection that can lead to organ failure and death. Definitive antibiotic therapy, guided by culture and sensitivity results, is crucial but often remains suboptimal. Methods: This retrospective observational study analyzed medical records of adult sepsis patients from January to December 2024 at RSUD Prof. Dr. Margono Soekarjo. Antibiotic appropriateness was assessed based on culture results, susceptibility testing, and guidelines (PNPK 2017, IDSA 2020, DIH 2021). Data were analyzed using Chi-Square at a 5% significance level. Results: Of 77 regimens (133 antibiotics), 64.95% were combination therapies, mostly ceftazidime and levofloxacin. Appropriateness based on culture was 41.66%, guideline 37.66%, and dosage 75.94%. Clinical outcomes improved more in appropriate therapy groups, but not significantly (p > 0.05). Conclusion: Definitive antibiotic appropriateness remains low, despite high dosage conformity. Continued evaluation is essential to improve outcomes and prevent resistance. Emerged in the P. citrinum K6A fungal strain. Based on the literature studied, 6 of 11 predicted compounds have the potential as antifungal, antibacterial, and/or antiviral, including equisetine which possessed all those activities. Thus, P. citrinum K6A can be further explored through transcriptomic studies to determine the expression of the gene.
