Evaluasi Dosis Obat Antihipertensi pada Pasien Rawat Jalan di Puskesmas Sokaraja 1

Prevalensi hipertensi terus meningkat secara global, termasuk di Kabupaten Banyumas. Peningkatan hipertensi harus ditangani dengan tepat, salah satunya dengan evaluasi penggunaan antihipertensi. Ketidaksesuaian dosis menyebabkan inefektivitas terapi atau peningkatan risiko efek samping, sehingga mempengaruhi kualitas hidup pasien. Tujuan penelitian adalah mengevaluasi dosis berdasarkan PNPK Tata Laksana Hipertensi Dewasa 2021 dan Drug Information Handbook. Penelitian ini merupakan studi non-eksperimental observasional deskriptif secara prospektif. Data dikumpulkan melalui analisis rekam medis pasien rawat jalan di Puskesmas Sokaraja 1. Sampel adalah pasien berusia 19-75 tahun, pasien prolanis dengan atau tanpa komorbid. Hasil dari 91 pasien menunjukkan 51,64% pasien menerima monoterapi dan 48,36% menerima politerapi. Penggunaan antihipertensi paling banyak diresepkan adalah amlodipin. Evaluasi peresepan menunjukkan dosis antihipertensi yang diberikan sesuai dengan literatur sebanyak 87 pasien (95,60%) dan yang tidak sesuai sebanyak 4 pasien (4,40%). Tingginya tingkat kesesuaian dosis ini menunjukkan bahwa tenaga kesehatan di Puskesmas Sokaraja 1 telah menerapkan terapi antihipertensi dengan baik dan sesuai standar.

The global prevalence of hypertension continues to rise, including in Banyumas Regency, Indonesia. This increasing trend requires appropriate management strategies, one of which is the evaluation of antihypertensive drug use. Inappropriate dosing may result in suboptimal therapeutic outcomes or increased risk of adverse effects, ultimately affecting patients’ quality of life. This study aimed to evaluate the appropriateness of antihypertensive dosing based on the 2021 National Clinical Practice Guidelines for the Management of Adult Hypertension (PNPK) and the Drug Information Handbook. A prospective, non-experimental, observational descriptive study was conducted using outpatient medical records from Sokaraja 1 Primary Healthcare Center. The study included hypertensive patients aged between 19 and 75 years, enrolled in the Prolanis program, with or without comorbid conditions. A total of 91 patients were evaluated, of whom 51.64% received monotherapy and 48.36% received combination therapy. Amlodipine was the most frequently prescribed antihypertensive agent. Dose evaluation showed that 87 patients (95.60%) received medications in accordance with guideline-recommended dosages, while 4 patients (4.40%) received doses outside the recommended range. The high rate of dose conformity indicates that healthcare providers at Sokaraja 1 Primary Healthcare Center have successfully implemented antihypertensive therapy in accordance with national and international standards.
