Latar Belakang: Hipertensi merupakan kondisi kesehatan jangka panjang dan penyebab utama kematian dini diantara orang dewasa di seluruh dunia. Evaluasi penggunaan obat merupakan proses evaluasi terstruktur dan berkesinambungan yang bertujuan mendapatkan gambaran pola penggunaan obat di fasilitas pelayanan kesehatan, maka dari itu peran apoteker sangat penting untuk melaksanakan evaluasi penggunaan obat agar tercapainya terapi yang aman, efektif, dan efisien bagi pasien.

Tujuan: Penelitian bertujuan untuk mengetahui kesesuaian penggunaan obat antihipertensi pada pasien hipertensi berdasarkan tepat indikasi, tepat pasien, tepat obat, dan tepat dosis.

Metodologi: Penelitian ini marupakan penelitian observasional dengan metode deskriptif menggunakan teknik pengumpulan data secara retrospektif dari rekam medik pasien hipertensi pada Januari-Maret 2020. Rasionalitas yang diteliti yaitu tepat indikasi, tepat pasien, tepat obat yang dianilisis berdasarkan PERKI 2015 dan tepat dosis yang dianalisis berdasarkan ACC/AHA 2017.
Hasil Penelitian: Jumlah sampel yang digunakan adalah  300 data rekam medik dari bulan Januari-Maret 2020. Obat antihipertensi yang digunakan amlodipine dan captopril. Hasil penelitian rasionalitas penggunaan obat antihipertensi di Puskesmas Kedungwuni I menunjukkan tepat indikasi 100%, tepat pasien 100%, tepat obat 64,67% dan tepat dosis 64,67% .
Kesimpulan: Dari hasil penelitian dapat disimpulkan bahwa untuk meningkatkan ketepatan obat dan dosis dapat dilakukan pemantauan lebih lanjut dalam ketersediaan obat antihipertensi di fasilitas kesehatan.

Background: Hypertension is a long-term health condition and a leading cause of premature death among adults worldwide. Evaluation of drug use is a structured and continuous evaluation process that aims to get an overview of the pattern of drug use in health care facilities, therefore the role of pharmacists is very important to carry out evaluations of drug use in order to achieve safe, effective, and efficient therapy for patients.
Objective: This study aims to determine the suitability of the use of antihypertensive drugs in hypertensive patients based on the right indication, the right patient, the right drug, and the right dose.
Methodology: This study is an observational study with a descriptive method using retrospective data collection techniques from medical records of hypertensive patients in January-March 2020. The rationales studied were the right indication, right patient, right drug analyzed based on PERKI 2015 and right dose analyzed based on ACC/AHA 2017.
Research Results: The number of samples used was 300 medical record data from January-March 2020. The antihypertensive drugs used were amlodipine and captopril. The results of the research on the rationality of the use of antihypertensive drugs at the Kedungwuni I Public Health Center showed that 100% correct indication, 100% correct patient, 64.67% correct drug and 64.67% correct dose.
Conclusion: The results of the research can be concluded that to improve the correct drug and correct dose, it needs further monitoring of the availability of antihypertensive drugs in health facilities.
