Latar Belakang : Gagal ginjal merupakan penyakit katastropik yang banyak menghabiskan biaya. Pemerintah menanggung pembiayaan pengobatan berdasarkan Tarif INA CBGs, akan tetapi sering terjadi perbedaan biaya pada pelayanan dengan Tarif INA CBGs. Penelitian ini bertujuan untuk mengetahui perbedaan biaya medis langsung dengan Tarif INA CBGs serta mengetahui faktor yang mempengaruhi perbedaan biaya medis langsung hemodialisa rawat jalan di RSD Gunung Jati Kota Cirebon.
Metodologi : Penelitian ini merupakan penelitian analitik cross sectional dilakukan pada Desember 2021 hingga Februari 2022. Sampel penelitian ini diambil menggunakan teknik random sampling. Jumlah sampel penelitian ini sebanyak 77 pasien dengan 3.392 episode hemodialisa rawat jalan. Analisis data yang digunakan adalah analisis deskriptif, Uji Mann Whitney, Uji Kruskal Wallis dan Uji Korelasi Bivariat Spearman.
Hasil Penelitian : Rata-rata biaya medis langsung pelayanan hemodialisa di RSD Gunung Jati Kota Cirebon sebesar Rp 633.075 dengan biaya tertinggi terdapat pada biaya alat dan bahan habis pakai dengan rata-rata sebesar Rp 326.263 per pasien. Terjadi selisih positif antara biaya medis langsung dan Tarif INA CBGs sebesar Rp 246.025 per tindakan hemodialisa rawat jalan. Faktor yang mempengaruhi perbedaan biaya medis langsung adalah penyakit komplikasi dengan perbedaan biaya sebesar Rp 380.787 pada pasien hemodialisa dengan penyakit komplikasi.
Kesimpulan : Terdapat selisih positif pada biaya medis langsung dengan Tarif INA CBGs pada pelayanan hemodialisa rawat jalan di RSD Gunung Jati Kota Cirebon. Faktor yang mempengaruhi perbedaan biaya medis langsung adalah penyakit komplikasi. Berdasarkan keuntungan tersebut, rumah sakit perlu melakukan penambahan alat dan fasilitas sehingga akan lebih banyak pasien yang melakukan pelayanan hemodialisa.

Background : Kidney failure is a catastrophic disease that costs a lot of money. Indonesian government covers the treatment financing through the INA CBGs Rates, however, most often the cost has the difference. This study aims to analyze the differences between direct medical costs and INA CBG rates and to find out the factors that affect the difference in direct medical costs of outpatient hemodialysis at Gunung Jati Hospital Cirebon.
Methodology : This research is a cross-sectional analytical study from December 2021 until February 2022. The sample of this study was taken using a random sampling technique and had 77 patients with 3.392 episodes of outpatient hemodialysis. The analysis in this research used descriptive statistics, Mann-Whitney test, Kruskal-Wallis test, and Spearman Bivariate Correlation.
Results : The average direct medical cost of hemodialysis services at Gunung Jati Hospital in Cirebon is Rp. 633,075 with the highest costs found in consumable tools and materials cost with an average of Rp. 326,263. There is a positive difference between direct medical costs and INA CBGs rates of Rp 246.025 for one outpatient hemodialysis procedure. The Factor that affects the difference in direct medical costs is complication disease, with a cost difference of Rp 390.787 for hemodialysis patients with complication disease.
Conclusions : There is a positive difference in direct medical costs with INA CBG rates in outpatient hemodialysis services at Gunung Jati Hospital in Cirebon. The factor affecting the difference in direct medical costs is a complication disease. Based on the profit, the hospital needs to add the tools and facilities that could increase the patients that served.
