Latar belakang: Masih adanya kasus dan kematian akibat leptospirosis di Daerah Istimewa Yogyakarta (DIY) membuat perlu dilakukan peningkatan kewaspadaan dan kesiapsiagaan kemungkinan terjadinya kejadian luar biasa (KLB) dalam bentuk Sistem Kewaspadaan Dini dan Respon (SKDR). Indikator pelaksanaan SKDR berupa ketepatan dan kelengkapan laporan dari seluruh puskesmas. Berdasarkan laporan SKDR DIY tahun 2018 ketepatan waktu pelaporan di Kabupaten Sleman dibawah capaian nasional. Penelitian ini dilakukan untuk mengevaluasi sistem program SKDR penyakit leptospirosis di Kabupaten Sleman berdasarkan aspek input, proses, dan output.
Metode: Penelitian ini menggunakan pendekatan kuantitatif dengan metode penelitian survei yang bersifat deskriptif. Penelitian ini bertujuan mengevaluasi  program yang sedang berjalan dan hasil yang telah dicapai. Analisis data menggunakan pendekatan analisis univariat.
Hasil Penelitian: Berdasarkan hasil penelitian masih ditemukan beberapa masalah pada aspek input seperti tugas rangkap, kurangnya sumber daya pelaksana SKDR, tidak tersedianya alokasi dana, dan tidak tersedianya pedoman pelaksana SKDR. hal tersebut mempengeruhi proses pelaksanaan surveilans SKDR penyakit leptospirosis di beberapa puskesmas
Kesimpulan: Beberapa variabel dalam aspek input, proses, dan output tidak sesuai dengan pedoman pelaksanaan SKDR. Dinkes Kabupaten Sleman diharapkan terus berkoordinasi dengan Dinkes Provinsi dan puskesmas untuk bekerja sama dalam mengatasi permasalahan yang ada dari aspek input, proses, dan output.

Background: There are still cases and deaths due to leptospirosis in the Special Region of Yogyakarta (DIY) making it necessary to increase awareness and preparedness for the possibility of an outbreak in form of a Sistem Kewaspadaan Dini dan Respon (SKDR). Indicators of SKDR implementation in form of punctuality and completeness of weekly reports from all public health center. Based on DIY SKDR report in 2018, the timeliness of report submission in Sleman is below national achievements. This study is conducted to evaluate SKDR program for leptospirosis in Sleman Regency based on input, process, and output aspects.
Method: This research uses a quantitative approach with descriptive survey research methods. This study aims to evaluate the ongoing program and the already achieved results. Also, univariate analysis method is used in the data analysis.
Results: Based on the study, there are still some problems in input aspect such dual tasks, lack of SKDR implementing resources, lack of funds allocation, and lack of SKDR implementing guidelines. It affects the process aspect of SKDR surveillance of leptospirosis at some public health center. 
Conclusion: There are several variables in the aspects of input, process, and output which are not in line with the SKDR implementation guidelines. Department of Health of Sleman should coordinates continuously with Provincial Department of Health and local public health center to work together to overcome the current problems in input, process, and output aspects.
