Jaminan persalinan atau dikenal sebagai Jampersal merupakan salah satu kebijakan pembiayaan yang dilaksanakan oleh Kementerian Kesehatan Republik Indonesia yang bertujuan untuk meningkatkan akses pemeriksaan kehamilan, persalinan, pelayanan nifas dan bayi baru lahir yang dilakukan oleh tenaga kesehatan dengan menghilangkan hambatan finansial dalam rangka menurunkan Angka Kematian Ibu dan Angka Kematian Bayi. Tanggapan pemerintah kabupaten atau kota terhadap program Jampersal sangat bervariasi, ada daerah yang menerima dan ada yang tidak. Puskesmas Kutawis merupakan salah satu Puskesmas di Kabupaten Purbalingga yang berhasil mencapai target cakupan K1 dan cakupan K4. Proporsi pertolongan persalinan menggunakan jampersal di Puskesmas Kutawis juga tinggi yaitu 97,4%. Keberhasilan Puskesmas Kutawis dalam mencapai target cakupan K1 dan K4 serta tingginya pengguna Jampersal di Puskesmas tersebut yang melatar belakangi peneliti untuk melakukan penelitian di lokasi tersebut. Penelitian ini mendeskripsikan faktor apa yang menghambat proses implementasi kebijakan Jampersal dalam pelayanan ANC. Dalam menganalisa data temuan peneliti menggunakan teori yang mempengaruhi implementasi kebijakan menurut George C. Edwar III. Hasil penelitian menunjukan bahwa implementasi kebijakan jampersal di puskesmas kutawis belum berjalan dengan baik. Implementasi kebijakan terhambat pada komunikasi yang belum berjalan baik terlihat dari pemahaman implementor yang sangat kurang tentang mekanisme pelaksanaan dan pelaporan kebijakan. Disposisi  implementor yang masih rendah, tercermin pada masih ada beberapa implementor yang belum menjalankan tugasnya sesuai dengan aturan yang ada. Struktur birokrasi terdiri dari beberapa bagian organisasi pelaksana yang belum terkoordinasi dengan baik antara puskesmas dengan Dinas Kesehatan Purbalingga.

ABSTRACT

Maternity insurance or it is known  as jampersal, is one of the  health financing policy   that  is conducted  by  the Ministry of Health of the Republic  of Indonesia that aims to  inrease   the access of prenatal care, delivery assistance, postpartum care, and newborn care   that   have  been   conducted  by the Health Officer by removing   the  financial  obstacle  in order to   diminish   the maternal mortality rate and inflant mortality rate. The respond  of regency  government  to the Jampersal policy is very difference, there is the region  that accept and not accept. Puskesmas of Kutawis is one of  the Puskesmas in the Regency of  Purbalingga that succeeds to achieve the   coverage target K1  and K4.  Assistance  proportion  of delivery uses  the Jampersal in the Puskesmas of Kutawis  is also high  that  is 97.4%. The success of Puskesmas of Kutawis in achieving the coverage target  K1  and K4  and the high  of Jampersal  user in Puskesmas, it becomes  the reason to do this research  in this location. The  type  of research is descriptive  by using qualitative research design. This study describes the factors that hinder the process of policy implementation Jampersal in the ANC. In analyzing the data using the theory to research findings that affect the implementation of policies by George C. Edward II. Analysis method in this resarch is based on trianggulation source. The result of research  shows  that the implementation of Jampersal policy in Puskesmas Kutawis has not run well. Implementation of policies hampered the communication has not gone well visible from the implementor of very poor understanding of the mechanisms of implementation and reporting policies. Disposition implementor is still low, reflected on the implementor there are still some who do not carry out their duties in accordance with the existing rules. The bureaucratic structure is composed of several parts of the implementing organization that has not been well coordinated between Puskesmas with the Health Department Purbalingga
