PERBEDAAN KEKERASAN DAN KEPADATAN GIGI WHITE SPOT LESION PASCA APLIKASI 5% NaF-TCP ANTARA ETSA ASAM FOSFAT 25% DAN 37% (Studi in vitro pada Gigi Premolar Muda)

Aplikasi fluoride varnish dosis tinggi setelah proses demineralisasi ulang dengan etsa asam merupakan treatment planning yang sesuai dengan karakteristik white spot lesion. Penelitian ini bertujuan untuk mengetahui perbedaan kekerasan dan kepadatan gigi white spot lesion pasca aplikasi 5% NaF-TCP antara etsa asam fosfat 25% dan 37%. Sebanyak 24 sampel gigi premolar muda (dari pasien berusia 10-16 tahun) post-ekstraksi ortodonsi dibagi menjadi 3 kelompok besar, yaitu: Kelompok I (aplikasi asam fosfat 25%+5% NaF-TCP ), Kelompok II (aplikasi asam fosfat 37% +5% NaF-TCP) dan Kelompok kontrol (aplikasi 5% NaF-TCP varnish saja). Masing-masing kelompok dibagi lagi dalam 2 sub-kelempok untuk pengujian kekerasan (A) dan kepadatan (B). White spot lesion berukuran 4x3 mm dibuat pada permukaan bukal sampel dengan cara perendaman dalam larutan demineralisasi Frazier selama 5 hari. Aplikasi etsa untuk demineralisasi ulang dilakukan selama 15 detik dengan 20 detik pembilasan. Aplikasi fluoride varnish menggunakan metode Knutson. Sampel kemudian disimpan dalam IMWS selama 14 hari. Kekerasan diukur dengan Microvickers Hardness Tester dan kepadatan diukur menggunakan gelombang suara 20 KHz/ 38dB dan dianalisa menggunakan software Cool Edit Pro 2.0. Uji Paired T-test dan Wilcoxon menunjukkan terdapat perbedaan kekerasan dan kepadatan yang signifikan sebelum dan setelah perlakuan (p=0,000 dan p=0,002). Rata-rata selisih peningkatan kekerasan dan kepadatan tertinggi didapatkan oleh kelompok I (503,5 Mpa, 288,54 Hz), diikuti kelompok III (497,5 MPa, 154,8 Hz) dan kelompok II (482,2 MPa, 64,4 Hz). Uji One Way ANOVA menunjukkan bahwa tidak terdapat perbedaan signifikan antara selisih kekerasan gigi pada ketiga kelompok (p=0,427). Uji Kruskall Wallis menunjukkan bahwa terdapat perbedaan signifikan selisih kepadatan gigi (p=0,029). Uji Mann-Whitney menunjukkan bahwa perbedaan yang signifikan terjadi antara kelompok I dan III dan kelompok II dan III. Aplikasi asam fosfat 25% menghasilkan peningkatan nilai kekerasan dan kepadatan lesi white spot lesion permanen muda, namun tidak signifikan dibandingkan dengan kelompok kontrol.

Acid etching followed by high-dosed fluoride application is a proposed mechanism to enhance the remineralization treatment of white spot lesion. The aim of this study was to determine the microhardness and density differences between white spot lesion on young premolar teeth etched by 25% and 37% phosphoric acid followed by 5% NaF-TCP treatment . 24 young premolar teeth extracted from 10-16 years old orthodontic patients were randomly divided into three large groups as follow: Group I (25% phosphoric acid+5% NaF-TCP treatment), Group II (37% phosphoric acid+5% NaF-TCP treatment) and Group III/Control Group (5% NaF-TCP treatment without acid etching). Each group was then subdivided into 2 groups: microhardness measurement (A) and density measurement (B). Artificial white spot lesions (4x3 mm) were produced on the bucal surface of the teeth by immersing in Frazier solution. The acid etchants were applied for 15 s, after which the samples were washed for 20 s. Knutson’s method was applied as fluoride varnish application technique. The samples were kept in IMWS for 14 days.The microhardness was determined with Microvickers Hardness Tester (25 g load for 15 seconds) and the density was determined with sonic waves (20 KHz/38 dB) and analyzed with Cool Edit Pro 2.0 software. Paired T-test and Wilcoxon revealed that there were statistically significant differences (p=0,000 and p=0,002) in microhardness and density before and after treatment. The maximum change in the measured parameters was seen in Group I (503,5 MPa, 288,54 Hz), followed by Group III (497,5 MPa, 154,8 Hz) and Group II (482,2 MPa, 64,4 Hz) respectively.One Way ANOVA revealed that no significant differences were found between the microhardness change in three groups (p=0,427). Kruskall Wallis revealed that there were statistically significant differences between the density change in three groups (p=0,029). Mann-Whitney revealed that the significant differences were found between Group 1 and 3 and between Group 2 and 3. Acid etching with 25% phosphoric acid resulted in enhancement of remineralization on white spot lesion, but the change wasn’t statisically significant compared to non-etching group.
